Perhaps one of the most telling incisive questions you could possibly ask someone, but only if you actually mean it and are actually prepared to receive the answer.
We use a strategy at the practice when we're looking after people: We say, "Are you okay? " Usually, the person replies, "I'm fine," and then we ask, "Are you really okay? "
Usually, that elicits a totally different kind of answer - I've written about that before.
But the most important thing is not to ask the question unless you're prepared to receive the answer.
Stephen Covey taught us that scheduling personal conversations is impossible, and we shouldn't even try.
So if you're asking if someone is ok, make sure you've got some time to listen.
